What We Do

Care Squared’s AEPs specialise in creating and delivering safe and effective exercise programs for participants at risk of, or diagnosed with disabilities, illnesses and conditions related to aging, and chronic medical conditions, including complex conditions.

We assess and evaluate how well you can perform activities required for your day-to-day life, devising a plan that encompasses your goals and optimises and improves your health, making it easier for you to engage in activities at home, school, work, and in the community.

Our end goal is to empower you to be able to better self-manage your health conditions and enjoy a better quality of life.

Our exercise physiologists work with participants in a number of settings in regional and city centres, including at home, school, community location or via telehealth. Our services are provided in line with the NDIS price guide, meaning there’s no gap fee.

Ways We Can Help:

  • Conduct an exercise pre-screening assessment to identify injury history, family history of any chronic conditions, and current lifestyle habits
  • Help you set goals and strategies, and empower you to self-manage your participation in physical activity
  • Develop exercise plans for recovery, management or prevention
  • Assist you with weight loss or maintenance
  • Provide intervention for a range of chronic and complex health conditions
